This form grants temporary guardianship by a parent or legal custodian to another adult so that they may properly care for a child(ren). This may be needed if the legal guardian of the child is unable to care for the child for any reason. A POA will allow the temporary guardian to enroll the child in school, handle Dr.'s visits, etc.
